What You Need
|
|
|
|
Your first appointment will be quicker if you are prepared with certain things. According to the Social Security Administration, you will need the following:
* Your Social Security number * The Social Security number of any dependants who will receive benefits * A certified copy of your birth certificate and, if you were born abroad, proof of US citizenship or legal residency * Original or certified military discharge papers * Names, addresses, and phone numbers of doctors, hospitals, clinics and other institutions where you have received medical care * Dates of all visits to any of these health care institutions * Your patient ID numbers * Names and dosages of all medications you are taking * All medical records related to your disability * A brief summary of the employers for whom you have worked in the past 15 years, including a brief work description for each employer and dates of employment * A copy of your most recent W-2 form or, if you were self-employed, your most recent federal tax return * Workers' compensation information * Banking account numbers * Contact information for another person if you are unavailable
Whether you come with us or go to the Social Security Administration office, you need to bring as many of these items as possible. If you are missing any of the documents we can assist you in gathering them.
In addition to the above information, it is important that you have a doctor's report that provides specific information important to proving your claim. The report you obtain from your doctor should have the following information and should be based on a recent medical examination:
* Diagnoses of injuries or illnesses that are causing the severe impairment of work capacity * Work restrictions that result from the impairment * Examples of activities that you can and cannot do * Medical opinion as to whether or not the disabling condition is expected to last at least 12 months or result in death
